1.MECHANISM OF ACTION

Plecanatide and its active metabolite bind to and activate guanylate cyclase-C (GC-C) receptors on the luminal surface of intestinal epithelial cells. This activation increases intracellular and extracellular cyclic guanosine monophosphate (cGMP), which stimulates fluid secretion into the intestinal lumen and accelerates bowel transit. The action is pH-dependent and primarily localized to the proximal small intestine, mimicking the physiological activity of endogenous uroguanylin to normalize bowel function.
