Guanylin binds specifically to the extracellular domain of guanylate cyclase-C (GC-C) receptors expressed on intestinal epithelial cells. This binding activates the intracellular catalytic domain of GC-C, leading to the conversion of GTP to cyclic GMP (cGMP). The elevated intracellular cGMP levels activate protein kinase G (PKG) and other downstream effectors, resulting in phosphorylation of the cystic fibrosis transmembrane conductance regulator (CFTR) chloride channel. This cascade ultimately stimulates chloride and bicarbonate secretion into the intestinal lumen, while inhibiting sodium absorption. The net effect is increased fluid secretion and altered electrolyte transport, which plays a critical role in maintaining intestinal fluid homeostasis and responding to enteric pathogens.
