Pasireotide exerts its therapeutic effects by binding with high affinity to somatostatin receptors (SSTR), specifically subtypes 1, 2, 3, and 5, which are overexpressed on pituitary adenoma cells. This binding inhibits the secretion of key hormones: in Cushing's disease, it reduces adrenocorticotropic hormone (ACTH) release, leading to decreased cortisol production; in acromegaly, it directly suppresses growth hormone (GH) secretion and normalizes insulin-like growth factor-1 (IGF-1) levels. Its unique broad receptor-binding profile, particularly its high affinity for SSTR5, differentiates it from older somatostatin analogs and underlies its efficacy in patients resistant to first-generation therapies.
