1.MECHANISM OF ACTION

Uroguanylin exerts its biological effects by binding and activating the guanylyl cyclase-C (GC-C) receptor, a transmembrane enzyme expressed predominantly on the apical membrane of intestinal and renal epithelial cells. This binding activates the receptor's intracellular catalytic domain, converting guanosine triphosphate (GTP) into the second messenger cyclic guanosine monophosphate (cGMP). Elevated intracellular cGMP then activates protein kinase G (PKG) and type II cGMP-dependent protein kinase (cGKII).

3.CLINICAL EFFICACY OF PRODUCT

Physiological Role: It is a key regulator of intestinal fluid secretion and renal sodium excretion. Its release postprandially is thought to link intestinal salt intake with renal salt excretion, a concept known as the "gut-renal axis."

Therapeutic Analogs: The understanding of uroguanylin's mechanism directly led to the development of stable GC-C receptor agonists:

Linaclotide (Linzess®) and Plecanatide (Trulance®) are FDA-approved drugs for chronic idiopathic constipation (CIC) and irritable bowel syndrome with constipation (IBS-C). These are structurally related, acid-stable analogs of uroguanylin/guanylin that mimic its action locally in the intestine.

Proven Clinical Efficacy: Linaclotide and plecanatide have demonstrated significant clinical efficacy in increasing bowel movement frequency, improving stool consistency, and reducing abdominal pain in large-scale clinical trials. They work by the same final pathway (GC-C/cGMP/CFTR activation) as uroguanylin.
